Insurance Plans
At Grapevine Physical Therapy and Sports Medicine we accept all major insurance plans and will be happy to bill your therapy claims to them. We will collect any applicable copays/deductibles as required by your plan provisions.
If we are not on your insurance plan, we will work with you and your out of network benefits.
Physical Therapy Consultation
The physical therapist can see you for a one-time consultation/assessment without a physician referral. Contact the clinic for information about a PT consultation.
Doctor Referrals
In the state of Texas, Physical Therapy cannot be practiced without a doctor's prescription. Contact the clinic for assistance with your doctor referral.
The First Appointment
- Arrive 15 minutes early to complete paperwork.
- Average first appointment lasts 1.5 hours. Subsequent appointments vary from 30-60 minutes

Payment Procedures
- Payment for therapy services are due at the time of the visit.
- We will file your insurance for you.
- We accept all major credit cards, checks and cash.
- Payment plans can be developed if necessary.
